Life in Balance is a 42-week community-based chronic care initiative led by the Florida Economic Consortium, Inc., a not-for-profit organization. This program focuses on underserved adults in Central Florida (Orange, Seminole, Volusia, and Lake counties) who are managing diabetes, hypertension, or high cholesterol.

The initiative’s ultimate goal is to improve long-term health outcomes while decreasing indigent care costs and preventing costly hospital visits or readmissions. Services are delivered both in-person and virtually, ensuring broader access and accommodating those with transportation or mobility barriers
