LIFE N BALANCE — A CHRONIC CARE INITIATIVE

Life N Balance is a chronic disease prevention and self-management program designed to help adults take greater control of their health. The program serves adults age 18 and older, including adults age 55 and older, who have been diagnosed with diabetes, hypertension, or high cholesterol or who are at elevated risk of developing a chronic disease.
Through health education, screenings, community health worker support, wellness coaching, nutrition education, physical-activity guidance, individualized health planning, and connections to healthcare and community resources, participants receive practical assistance for improving their health and managing chronic conditions more effectively.
PROGRAM PURPOSE
Diabetes, hypertension, and high cholesterol can lead to serious health complications when they are not properly monitored and managed. Individuals may understand that they have a chronic condition but still experience difficulty interpreting health information, following treatment recommendations, taking medications consistently, making nutritional changes, remaining physically active, or obtaining preventive care.
Other participants may face barriers involving access to healthcare providers, transportation, food, social services, affordable resources, or follow-up care. These challenges can contribute to poorly controlled health conditions, preventable complications, avoidable emergency-room visits, and hospitalizations.
Life N Balance addresses these needs by giving participants the knowledge, skills, resources, and continuing support needed to become more active participants in their healthcare. The program emphasizes prevention, early intervention, informed decision-making, and practical behavior changes that can be maintained over time.
PROGRAM CONCEPT
The central concept of Life N Balance is that effective chronic disease management occurs in everyday life, not only during medical appointments. Participants must make ongoing decisions about medications, food, physical activity, monitoring, appointments, and responses to changes in their health.
The program helps participants understand how those daily decisions affect their long-term health. Rather than relying solely on general health information, Life N Balance combines education with individualized goal-setting, routine screenings, home-monitoring instruction, coaching, referrals, and follow-up support.
Participants are encouraged to work with their healthcare providers and follow the treatment plans prescribed for them. Life N Balance does not replace medical care. It helps participants understand and carry out appropriate self-management practices between healthcare appointments.
CONDITIONS ADDRESSED
Life N Balance focuses primarily on diabetes, hypertension, and high cholesterol. These conditions may occur independently or together and can increase the risk of heart disease, stroke, kidney disease, vision problems, nerve damage, and other serious complications.
Participants learn what their conditions mean, why routine monitoring is important, and how medication use, nutrition, physical activity, preventive care, and consistent follow-up can affect their health.
The program also serves adults who are at elevated risk of chronic disease. Prevention education helps these participants recognize risk factors and begin making healthier choices before a serious condition or complication develops.
HEALTH EDUCATION
Health education workshops provide participants with understandable information about chronic conditions and the steps they can take to manage their health. Instruction is designed to help participants translate health recommendations into practical actions they can use at home and in their communities.
Educational topics include:
Understanding diabetes, hypertension, and high cholesterol.
Recognizing personal health risks.
Understanding the purpose of routine health monitoring.
Using medications correctly and consistently.
Communicating medication concerns to healthcare providers.
Making healthier food choices.
Reading and understanding nutrition labels.
Understanding portion sizes and meal planning.
Participating in safe and appropriate physical activity.
Recognizing warning signs that may require medical attention.
Understanding the importance of preventive care and follow-up appointments.
Participants learn how chronic conditions can change over time and why a condition may still require attention even when they do not feel sick. The program reinforces the importance of following professional medical guidance and seeking timely care when warning signs or significant health changes occur.
HEALTH SCREENINGS AND MONITORING
Life N Balance provides opportunities for health screenings at community-based locations, faith-based locations, health fairs, and outreach events. Available screenings include blood pressure checks, blood glucose monitoring, A1C screenings, and cholesterol screenings.
An initial health assessment helps identify each participant’s existing conditions, health concerns, risk factors, current self-management practices, and potential barriers to care. Screening results may be used to help participants better understand their health status and determine whether additional medical evaluation or follow-up is needed.
Routine screenings also provide participants with opportunities to observe changes in important health measures. Participants receive guidance on the meaning of their results and are referred to appropriate healthcare providers when findings require professional medical attention.
The program also teaches participants how to monitor their health at home when appropriate. Home-monitoring instruction may include the proper use of blood pressure or blood glucose equipment, maintaining records, recognizing unusual results, and knowing when to contact a healthcare provider.
INDIVIDUAL HEALTH ACTION PLANS
Participants may develop individual health action plans based on their needs, health conditions, screening information, available resources, and personal goals.
Health action plans may address:
Taking medications as directed.
Monitoring blood pressure or blood glucose.
Keeping medical appointments.
Following up on screening results.
Making gradual nutritional improvements.
Increasing safe physical activity.
Recognizing warning signs.
Accessing healthcare or community resources.
Addressing barriers that interfere with care.
The purpose of an action plan is to turn broad health recommendations into realistic and manageable steps. Participants are encouraged to establish goals that reflect their circumstances and can be monitored over time.
Progress is reviewed through regular community health worker contact, follow-up after screenings, healthcare referrals, monitoring results, and discussions about the participant’s experiences carrying out the plan.
COMMUNITY HEALTH WORKER SUPPORT
Community health workers are a central part of Life N Balance. They provide education, coaching, encouragement, follow-up, and connections to services while helping participants better understand and navigate available health resources.
Community health worker responsibilities include:
Conducting needs assessments.
Helping participants establish health goals.
Providing chronic disease education.
Supporting medication adherence.
Reinforcing nutrition and physical-activity goals.
Following up after health screenings.
Connecting participants with healthcare providers.
Providing referrals to food assistance and social services.
Helping participants identify and address barriers to care.
Monitoring progress and encouraging continued participation.
Community health workers help bridge the space between healthcare recommendations and the participant’s daily circumstances. For example, a participant may understand the need to eat healthier but have limited access to nutritious food. Another participant may need medical follow-up but experience difficulty locating a provider or understanding where to obtain care.
The community health worker helps identify these barriers and connect the participant with appropriate healthcare, food, or social-service resources. This support is intended to improve access, encourage follow-through, and reduce the likelihood that health needs will remain unaddressed.
MEDICATION ADHERENCE
Medication-adherence education helps participants understand the importance of taking medications according to their healthcare provider’s directions.
Participants receive education about maintaining medication schedules, keeping an updated medication list, requesting refills before medications run out, and communicating questions or side effects to a qualified healthcare professional. Participants are instructed not to change dosages or discontinue prescribed medications without consulting the appropriate healthcare provider.
Community health workers may help participants identify barriers that affect adherence, such as difficulty understanding instructions, forgetting doses, obtaining refills, accessing a pharmacy, or communicating with a provider. When necessary, participants may be directed back to their healthcare provider or pharmacist for professional medication guidance.
NUTRITION AND PHYSICAL ACTIVITY
Nutrition education helps participants understand how food choices can affect blood glucose, blood pressure, cholesterol, body weight, energy, and overall health.
Participants learn to read food labels, identify healthier options, understand portion sizes, reduce excessive sodium, sugar, and unhealthy fats, and make practical food choices that support their individual health goals.
The program recognizes that healthy eating recommendations must be realistic. Participants may have different cultural food traditions, household responsibilities, financial resources, cooking abilities, and access to healthy foods. Education and coaching focus on manageable improvements rather than unrealistic or temporary restrictions.
Physical-activity education emphasizes safe movement appropriate to the participant’s health status and abilities. Participants learn how regular activity can support cardiovascular health, blood pressure control, glucose management, mobility, and overall well-being. Individuals are encouraged to consult their healthcare providers before beginning or significantly changing an exercise routine when medically appropriate.
PROGRAM DELIVERY
Life N Balance services are delivered through community-based locations, faith-based locations, health fairs, and community outreach events. These settings allow the program to bring health education, screenings, and support directly into communities where participants live, gather, and receive services.
Program activities may include group health education workshops, individual coaching, health screenings, needs assessments, goal-setting, referrals, follow-up contacts, and assistance connecting with healthcare and community resources.
Community-based delivery reduces reliance on participants independently locating and navigating services. It also creates opportunities to reach individuals who may not routinely participate in preventive healthcare.
PROGRESS MONITORING
Participant progress may be monitored through:
Initial health assessments.

Blood glucose or A1C monitoring.

Clinical measures may improve at different rates and are affected by multiple factors. The program therefore considers both health measurements and self-management behaviors when reviewing progress.

LONG-TERM GOAL
The long-term goal of Life N Balance is to help participants become informed, confident, and active managers of their health. By combining chronic disease education, health screenings, individualized planning, community health worker support, preventive care, referrals, and ongoing follow-up, the program provides a comprehensive framework for improving health behaviors and reducing preventable complications.
Through stronger self-management skills and better connections to healthcare and community resources, participants are supported in achieving improved health outcomes, a better quality of life, and reduced dependence on emergency care for health needs that can be addressed through prevention, monitoring, and timely treatment.
